Terrain Medical Membership

All 150 founding memberships lock in $500 a month for as long as they stay. Once the practice fills, new families join a waiting list and enroll at the standard rate of $600 a month.

Your whole family can join under one roof. Kids under 10 are free. A spouse or partner joins under your membership at $400 a month at the founding rate. Ages 10 to 17 are $300 a month for the first child, $200 for the second, and $100 for each after that. Every paid rate rises $100 once the practice fills. Children are seen from birth; anything that needs a hands-on exam is referred to a trusted local provider.

The founding rate stays open until every founding spot is filled.

More expensive insurance plans mostly just lower your hospital bills. They don’t buy you more time with a doctor.

A whole year of conventional primary care adds up to about 45 minutes of your doctor’s time, three visits at 15 minutes each. Your first appointment at Terrain Medical runs longer than that on its own, and every visit after it is included.

If you see specialists often, pair the membership with a higher-coverage insurance plan. If you rarely do, a basic plan may be all you need.

One thing to be clear about: Terrain Medical is not health insurance. I’m your doctor, not your insurance plan. Keep an insurance plan for hospitals, emergencies, and surgeries. My membership works alongside it.

What labs cost here.

The Terrain Lab Panel is included in your membership twice a year. And when something on your labs needs to be checked again, that repeat test is free too. For anything beyond that, you pay the lab or imaging center directly at wholesale rates, or run it through your insurance when that’s cheaper. There’s no markup from this practice. A few examples:

Same idea as labs. When you need a scan, I send you to a cash-pay imaging center and you pay them directly at their cash price. No markup from this practice.

How this compares to other options.

A doctor who knows you and has the time to find what’s wrong has usually meant concierge medicine. Conventional concierge practices charge a membership and still run your care through insurance, so the premiums, deductibles, and copays continue on top. Functional and integrative clinics charge a membership too, but many can’t prescribe or manage your full primary care, so you keep a separate doctor for that. Terrain Medical is both at once, primary care and integrative care from one physician, at $500 a month, with no insurance billing at all.

A few important details.

Should I keep my insurance?

Yes. Keep at least a catastrophic or high-deductible plan for hospital stays, surgery, emergency room visits, and specialist visits. The membership doesn't cover those; your insurance does. Bronze and catastrophic plans also qualify you for an HSA under the 2026 rules, which you can put toward labs, prescriptions, and other care.

Medicare

Membership isn't open yet to patients who are on Medicare or old enough to qualify for it. That changes in late 2027.

Annual payment option

Members who pay annually save 10 percent. At the founding rate, that's $5,400 for the year instead of $6,000, a savings of $600. At the standard rate, it's $6,480 instead of $7,200, a savings of $720.
